Angie, are you talking about check off list for orientation, or duty roster type. Do you have a copy of the nurse practice act from your state ? You will have to start with that as it differs from state to state. If you let me know what you are specifically looking for I will be happy to share.

looking for a simple check off list (ie:foley cath care, sterile technique, med pass, gt's etc) -- i haven't found one yet in a time crunch, might have to write one myself but i didn't want to miss something -- it'll be similar to a cna skills checkoff but for lvn's -- thank you, angie repka, rn

Your State Health Care Association would probly have something available if your facility is a member facility.
If you have any contacts in the teaching field at any of your local nursing schools, they might be able to help with a ready-made check list.
Both source would likely have checklists for both LVN/LPN & CNAs.
